Information
At your scheduled appointment time your therapist will briefly review the new patient form to make sure we have all information required, you may also be asked if you have any questions regarding your appointment. If you do not have any questions, you can proceed with the purpose of your visit.
Appointments
Services are by appointment only. The time for your appointment is reserved exclusively for you and cannot be given to someone else.
Cancellations
Repeated cancellations, without adequate notice, may result in not booking further appointments. Please cancel your appointment 24 hours prior, to a) not incur a late fee, b) allow the therapists to schedule someone else.
Emma Cancellation Policy:
Clients can cancel or reschedule an appointment anytime, as long as they provide 24 hours notice. If you cancel an appointment with less than 24 hours notice, or fail to show up, you will be charged for the appointment.
Change of Your Information
It is important that we have your most current information on file. If you change your name, address, telephone number please be sure to provide your new information in the client portal as soon as possible.
Notice of Privacy Practice Policies
Emma Platforms Inc recognizes a patient’s right to privacy and will not share their personal details with any 3rd party for any reason. Emma Platforms Inc provides each patient with a notice of the practice’s privacy policy that contains the elements required by HIPAA.

When filling in the appointment request form, you consent to being contacted via phone, email or text message. You can unsubscribe at any time form these lists. Your contact information will never be sold or shared with a 3rd party.
